2009 SMDEP Recommendation Form (PDF, 2 pages)

Letters of recommendation should come from either a pre-health advisor or two of your college professors (one must be from your major field of study).

The following is the process for submitting recommendations:

  1. Provide each pre-health advisor/professor with a copy of the 2009 SMDEP Recommendation Form (PDF, 2 pages). Be sure to complete the Applicant Information section of the form before giving it to your reference.

  2. Provide each pre-health advisor/professor with the SMDEP mailing address:

    Attn: Recommendations
    Association of American Medical Colleges
    2450 N Street, NW
    Washington, DC 20037-1127

  3. Follow-up with each reference to ensure that the recommendations were sent.

Please Note

If your references choose to submit a letter of recommendation on school letterhead, please have them attach the first page of the SMDEP Recommendation Form. While we will accept letters of recommendation without the form, it does severely delay forwarding the recommendation to your selected program(s).

If you receive a notice indicating that the SMDEP National Program Office does not have your letters of recommendation on file, it is your responsibility to follow-up with your references.
